The difference, briefly
An underarm pair puts the load into your upper body through a cradle under the arm. It is what most people are handed, and there is nothing to learn.
A forearm pair puts the load through your hand, with a cuff around the forearm holding the crutch on you. Because it stays on your arm, you can open a door or take out a wallet without putting it down, and most people take a day or so to get used to the different balance.
